We will collaborate with local services to support integration, growth and expansion, in order to provide a fully accessible and equitable experience across the UK.
To achieve this we will:​

Replicate
the Kentown model of community palliative care to reach more children with life-limiting conditions.
Create
sustainable teams around the child and family while demonstrating the impact of the Kentown model.
Equip
professionals and parents/carers with the knowledge and expertise to deliver the best health and social care for children with life-limiting conditions.

We focus on three key elements: nursing care, family support and service coordination. This wrap-around support helps families easily access all the help and assistance they need in their local communities.

During our pilot programme, we had tremendous feedback about the positive difference this support is making to families going through incredibly tough times.
As Kentown Support grows and expands into new areas of the UK, we continue to develop and adjust how we roll out this model of care so that it complements any established statutory and voluntary services in the area. Together, this support can make a critical difference to families’ lives.​​
