Our Programmes
The community focused model of our programmes has three key complementary elements: Nursing Care, Family Support and Service Co-ordination.
The Kentown Children's Palliative Care Programme is a new and ground breaking transformative initiative designed to reach the growing numbers of children and young people with life-limiting conditions. It ensures that every family gets the right care and support when and where they need it.

We work with and support already established teams to further their capacity to provide equitable access and levelling up of provision. We will put in new teams where required as well as building on the support we can offer families.

In addition, the programme aims to raise awareness so families know what help is available locally and can access listening and signposting support, find out more about housing, financial benefits and legal advice and any support grants they may need.
The Kentown Programme invests in supporting professionals who care for children with life-limiting conditions through an ongoing programme of education and training to increase their knowledge and awareness of children’s palliative care. By creating a greater awareness amongst professionals, the programme aims to ensure that more children and families will be successfully referred to the right services and support networks.

The Programmes, branded as Kentown Support are delivered by Children’s Palliative Care Community Nurse Specialists, Family Support Workers and Family Service Coordinators.
