Our Programmes

 
The community focused model of our programmes has three key complementary elements: Nursing Care, Family Support and Service Co-ordination. As we move into new areas we will be adding in additional elements such as therapeutic counselling.



 

The Kentown Children's Palliative Care Programme is a new and ground breaking transformative initiative designed to reach the growing numbers of children and young people with life-limiting conditions from birth to age 19. It ensures that every family gets the right care and support when and where they need it. ​​​

The Kentown programme fills a critical gap in the care and support for seriously ill children and their families. It adds clear value by complementing existing provision and offers important lessons for future integration and scaling of children’s palliative care services nationally. 

​

The programme raises awareness so families know what help is available locally, can  find out more about housing, financial benefits and legal advice and any support grants they may need.

 

The Kentown Programme invests in supporting professionals who care for children with life-limiting conditions through an ongoing programme of education and training to increase their knowledge and awareness of children’s palliative care. By creating a greater awareness amongst professionals, the programme aims to ensure that more children and families will be successfully referred to the right services and support networks.

The Programmes, branded as Kentown Support are delivered by Kentown Children’s Palliative Care Community Nurse Specialists, Family Support Workers, Family Service Coordinators and Counsellors.

Eligibility Criteria

Eligibility for the Kentown Programme is based on clinical need and includes children and young people from birth to 19 years of age who are living with life-limiting or life-threatening conditions.

Other family members including siblings and grandparents are eligible for wider support because we recognise that caring for a seriously ill child impacts the whole family.
